You've probably done this before

Most men who come to us have already tried. Often several times.

The six-month stretch where you trained four days a week and cut the beers. The app that counted everything. The diet a mate swore by. Some of it worked for a while. Most of it stopped working, and the weight came back, and the conclusion you drew was that you didn't have the discipline.

That's usually the wrong conclusion.

A commercial weight loss programme is built around a structured calorie deficit and behaviour change. For some people, that works well. For a lot of men over 40 it doesn't — because the underlying biology has changed, and the deficit alone isn't enough to overcome it.

A medically supervised approach starts from a different question. Not how do we push harder? Just: why isn't this working?

Why weight and BMI tell you almost nothing

Two men can stand on the same scales, return the same number, and have completely different health.

Weight can't tell you how much of you is muscle and how much is fat. It can't tell you where the fat sits - and that matters, because fat stored around the organs behaves very differently from fat stored under the skin. It can't tell you what your blood sugar has been doing over the last three months, whether your liver is under strain, or whether your thyroid is contributing.

BMI has the same limitation. It's a ratio of height to weight. It was never designed to describe an individual's health, and it routinely misclassifies men who carry significant muscle.

None of which means weight is irrelevant. It means weight is a single data point, and men have been asked to make big decisions off it for a very long time.

The information that's actually useful is in your blood.

Blood sugar regulation - the overlooked driver

Impaired blood sugar regulation is widespread in men over 40 and often develops years before it produces obvious symptoms. It's one of the strongest drivers of weight that won't shift, energy crashes through the day, and a hormonal picture that's harder to correct.

HbA1c gives a picture of average blood sugar across roughly the previous three months - a more stable measure than a single fasting reading, and one that reflects the pattern rather than the moment. Read alongside lipids, liver and thyroid function, it builds a considerably fuller account than weight or BMI alone.

Who is a candidate in Australia

It's reasonable to seek an assessment if:

* Your weight has climbed steadily despite genuine, consistent effort

* Energy drops away through the afternoon in a way it didn't five years ago

* There's a family history of type 2 diabetes, heart disease or stroke

* Other things have changed at the same time — sleep, motivation, recovery, libido

* A previous blood test flagged something and nothing ever followed from it

* You simply want to know where you stand, with real data rather than a guess

You don't need to be at a crisis point. Earlier is generally better, because the things this panel looks at develop quietly over years.

These are read together and in the context of your history, current medications and individual risk factors — not as isolated numbers. Where the clinical picture warrants it, your doctor may arrange additional testing beyond the standard panel.

What those results actually tell you

HbA1c reflects the pattern rather than the moment. A single fasting glucose reading tells you about that morning; HbA1c tells you what the last three months have looked like. It's the most useful early indicator of how your body is handling blood sugar, and changes here often precede any symptom you'd notice.

Lipid profile describes cardiovascular risk that builds silently for decades. Most men have had one done at some point and never had it explained in the context of everything else.

Liver function matters more in weight management than most men expect. Fat accumulation in the liver is common, frequently silent, and often improves considerably when the broader picture is addressed.

Kidney function and electrolytes establish baseline health and safety — necessary before any treatment conversation, and useful information in its own right.

Thyroid function is checked because thyroid conditions can contribute to weight change, fatigue and low mood, and are straightforward to identify once someone actually looks.

Pancreatic markers form part of the safety baseline and inform what's clinically appropriate for you.

The point of the panel isn't a diagnosis of "overweight." It's a picture of how your body is currently functioning — and a baseline you can measure against later.

How treatment decisions are made

There's no single treatment for weight that won't shift, because there's no single cause.

Where treatment is clinically appropriate, it follows from what the assessment finds and is directed at the drivers identified in your individual picture. That may involve addressing blood sugar regulation, cardiovascular risk, thyroid function, sleep, a review of medications you're already taking for other conditions, or a combination.

Eligibility is determined by clinical criteria - including BMI, existing health conditions, medical history and individual risk factors - assessed by a registered medical practitioner. It is prescribed only where clinically indicated, only following an individual assessment, and with appropriate monitoring built in.
